Required Documents:
- Application Form: Download at the bottom of this page. Fill out the details on a computer, print, and sign the form. Incomplete or handwritten forms will be rejected.
- Passport Copy: A photocopy of your passport info page.
- High School Certificate: Authenticated (see below).
- If you have diploma equivalent to High school education from Technical/Vocational university, you must additionally submit your last certificate from High school.
- We do not accept provisional certificates for graduation happened later than 3 years.
- The Ukraine – if you were exempt from final exams, ZNO/NMT is required.

- Transcript/Grade Report: Notarized official copy of the transcript from the last two years of high school, including subjects and marks (NOT hours).
- Bangladesh – if you cannot provide 11th and 12th year marksheet, you need to provide Secondary school certificate + SSC marksheet (10th year).
- Eligibility statement issued by your high school:
- The example of the statement will be provided by us upon your successful admission exam.
- Payment Confirmation: Fee for foreign education evaluation must be paid via your UIS account before you submit the documents for evaluation. See the “Payment for IE” section.
Document Verification:
Ensure all documents are authenticated according to the "verification of authenticity" guidelines section specific to the country where they were issued.
IMPORTANT NOTICE:
Starting April 1, 2024: All documents must have an original superlegalization sticker or an apostille in the format of the sticker attached.
In other words – you must submit the documents given to you by the Czech Embassy or your country´s Ministry if the authentication comes in the form of a sticker.
If you do NOT have superlegalization sticker or apostille sticker – this rule does NOT apply to you.
What if they authenticated original documents?
- If the competent authority in your country attaches stickers only to original documents , you must submit both:
- The document with the original sticker.
- A photocopy of the document above – front and back page
- Original Documents Return Options:
- Pick-up: Retrieve originals in person at the Foreign Diploma Evaluation Office upon scheduled agreement
- Mail: For applicants abroad, documents can be mailed back at your expense.
- Documents must be in English or Czech. Official translations from an approved sworn translator are required. Translations must be attached to the authenticated copies of the required documents inseparably (see the most common mistakes section).
Notarization:
- In case the new rules do not apply to you, you are advised to provide NOTARISED COPIES
- Summary of document verification: 1. Make a proper verification of authenticity according to the country the documents were issued => 2. Make a verified copy of your original documents => 3. IF necessary – Make an official translation of this verified copy (make sure the documents are firmly attached to each other).
Important Details:
- Ensure consistency in personal details (full name, date of birth, nationality) across all documents and your UIS system.
- Name changes must be supported by a notarized copy of the legal document (e.g., marriage certificate).
- If documents lack full personal details, a notarized statutory declaration/affidavit is required to verify identity (see the most common mistakes section).
Submission Process:
- After you pass the admission exam, you will receive information from us how to proceed with the internal evaluation for your study programme (exceptions apply).
Additional Information:
- Committee Evaluation: Documents fulfilling all criteria will be reviewed by the Committee. The evaluation process can be lengthy, and updates will be communicated via email as soon as we have any.
- Use and Retention of Documents: Documents submitted are for internal use only and will be retained by CZU; they will not be returned.
- Internal Evaluation Validity: Valid only for a single application in the current year. If unsuccessful, the internal evaluation process must be repeated for any future applications.
